This form is for premises where table eggs are brought to for grading and packing. It does not include premises or part of premises which only deal with eggs laid on the same premises.
The form also explains the conditions of designation and the restrictions that are likely to be applied if there is a disease outbreak.
The Template protocol is used to document how the egg packing centre will meet these conditions of designation.
