The completed GB Export Application Form should be returned to your Authorised Veterinary Practitioner (aPVP) or Export Assembly Centre (EAC) operator (as appropriate) at least 3 working days in advance of the proposed export.
For exports to GB, this form replaces the Traces Export Application Form. Where you see reference to its requirement, you should now use the GB Export Application Form below.
